HIPAA Privacy Notice — Effective Date: January 1, 2024
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
- Who We Are
Center for DNA Testing is committed to protecting the privacy of your health information. This Notice of Privacy Practices describes how we may use and disclose your protected health information (PHI) to carry out treatment, payment, and health care operations, and for other purposes permitted or required by law.
This notice applies to all records of your care generated by Center for DNA Testing, including all of our locations across the United States.
- Your Protected Health Information
Protected Health Information (PHI) is information about you, including demographic information, that may identify you and relates to your past, present, or future physical or mental health condition and related health care services, including DNA test results, biological samples, and medical history provided during the testing process.
- How We May Use and Disclose Your Information
We may use and disclose your protected health information in the following circumstances:
Treatment
We may use your PHI to provide, coordinate, or manage your DNA testing services and any related services. For example, we may share your information with laboratory partners who process your samples to deliver accurate results.
Payment
We may use and disclose your PHI to obtain payment for services provided to you. This includes billing, insurance verification if applicable, and processing payments through our secure payment systems.
Health Care Operations
We may use and disclose your PHI to support the business activities of Center for DNA Testing. These activities include quality assessment, employee training, accreditation, and conducting or arranging for other business activities.
Legal Requirements
We will disclose your PHI when required to do so by federal, state, or local law. This includes complying with court orders, subpoenas, or other lawful processes requiring disclosure of your information.
Chain of Custody — Legal Testing
For court-admissible DNA tests, your identity verification information and chain-of-custody documentation may be shared with the requesting court, legal authority, immigration body, or other authorized agency as required for the legal process.
Public Health Activities
We may disclose your PHI to public health authorities for activities authorized by law, including preventing or controlling disease, injury, or disability.
Serious Threats to Health or Safety
We may use or disclose your PHI when necessary to prevent a serious threat to your health and safety or the health and safety of the public or another person.
- Uses and Disclosures Requiring Your Authorization
Other uses and disclosures of your PHI not covered by this notice will be made only with your written authorization. You may revoke your authorization at any time in writing, except to the extent that we have already taken action in reliance on the authorization.
The following uses and disclosures will be made only with your written authorization:
- Most uses and disclosures of psychotherapy notes
- Uses and disclosures of PHI for marketing purposes
- Disclosures that constitute a sale of PHI
- Any use or disclosure not described in this notice
- Your Rights Regarding Your Health Information
You have the following rights regarding the PHI we maintain about you:
Right to Inspect and Copy
You have the right to inspect and obtain a copy of your PHI that may be used to make decisions about your care, including your DNA test results and related records. To request access, submit a written request to our Privacy Officer.
Right to Amend
If you believe that PHI we have about you is incorrect or incomplete, you may request that we amend the information. We may deny your request if the information was not created by us, or if we determine that the information is accurate and complete.
Right to an Accounting of Disclosures
You have the right to request a list of certain disclosures we have made of your PHI. This accounting will not include disclosures made for treatment, payment, or health care operations purposes.
Right to Request Restrictions
You have the right to request a restriction or limitation on the PHI we use or disclose about you for treatment, payment, or health care operations. We are not required to agree to your request unless required by law.
Right to Request Confidential Communications
You have the right to request that we communicate with you about your PHI in a certain way or at a certain location. For example, you may ask that we only contact you by email or at a specific address.
Right to a Paper Copy of This Notice
You have the right to a paper copy of this notice at any time, even if you have agreed to receive this notice electronically. Contact us at the information below to request a copy.
Right to Be Notified of a Breach
You have the right to be notified in the event that we discover a breach of your unsecured PHI. Notification will be provided to you without unreasonable delay and in no case later than 60 days following the discovery of the breach.
- Our Duties
Center for DNA Testing is required by law to:
- Maintain the privacy of your protected health information
- Provide you with this notice of our legal duties and privacy practices with respect to your PHI
- Notify you following a breach of your unsecured PHI
- Abide by the terms of the notice currently in effect
We reserve the right to change our privacy practices and the terms of this notice at any time, provided such changes are permitted by applicable law. We reserve the right to make the changes in our privacy practices and the new terms of our notice effective for all PHI that we maintain, including PHI we created or received before we made the changes.
- Complaints
If you believe your privacy rights have been violated, you may file a complaint with Center for DNA Testing or with the Secretary of the U.S. Department of Health and Human Services.
To file a complaint with us, contact our Privacy Officer using the information provided below. All complaints must be submitted in writing.
You will not be penalized, retaliated against, or discriminated against for filing a complaint.

- Effective Date and Updates
This Notice of Privacy Practices is effective as of January 1, 2024. Center for DNA Testing reserves the right to change this notice at any time. Any revised notice will be posted on our website at centerfordnatesting.com and made available at all of our locations.
